What is in the form
Course, Instructor, Overall, how would you rate this course (1 to 5), The instructor explained things clearly (1 to 5), Materials and readings were useful (1 to 5), Feedback on my work was helpful and timely (1 to 5), The pace was (one of 3), The difficulty was (one of 3), Hours per week you spent outside class (one of 4), What was the most useful part of the course (long answer), What should change next time it runs (long answer) and Would you recommend this course to another student (yes or no).
It ends with a thank-you message you can edit.
After someone submits
