What is in the form
Page 1: How much do you agree with each statement?. I know what is expected of me at work (one of 5), I have the tools and information I need to do my job well (one of 5), I receive recognition when I do good work (one of 5), My manager supports my development (one of 5), I can see a path to grow here (one of 5), My workload is manageable (one of 5), My opinions count and I can speak up safely (one of 5) and I would recommend this company as a place to work (one of 5).
Page 2: A little context. Your department (dropdown), How long have you been here (dropdown), What one thing would most improve your experience at work (long answer) and What should we make sure we keep doing (long answer).
It opens with a welcome screen (“Employee engagement survey”) and ends with a thank-you message you can edit.
After someone submits
This template comes without a workflow. Responses land in your dashboard, and you can add a step in the workflow editor: an approval by email, a post to Slack or Teams, a row in Notion or Airtable, or a webhook.
