Collect bug reports, feature requests and general feedback in one form, with steps to reproduce shown only for bugs and a Slack post for every submission.
What is in the form
What kind of feedback is it (one of 3), Which part of the product (dropdown), In one sentence, Tell us more (long answer), Steps to reproduce (long answer), Browser or device, How much does this matter to you (one of 3), Screenshot or recording (file upload) and Email, if we may follow up.
“Steps to reproduce” only appears when someone answers “Bug report” to “What kind of feedback is it?”. “Browser or device” only appears when someone answers “Bug report” to “What kind of feedback is it?”.
It ends with a thank-you message you can edit.
After someone submits
A Slack post. Every submission is posted to #product-feedback in your workspace, with the answers that matter pulled into the message, so the team sees “Product feedback” responses where they already are.
The workflow copies with the form, switched off until you have filled in the blanks. The form itself publishes on the free plan; switching an approval on is a Pro feature, and a Slack post needs your workspace connected once.
Make it yours
The options under “Which part of the product?”. are placeholders: put your own in. Add, remove or reorder questions in the builder; every change is tracked and nothing goes live until you publish. Pick a design preset or your own colours, add your logo, and share the link or embed the form on your site.
