Order form
A simple order form for a small shop or bake sale: quantities per item, delivery or pickup, a delivery address only when it is needed, and a Slack post for every order.
What is in the form
Your name, Email, Phone, Sourdough loaf (5.00), Cinnamon buns x4 (9.00), Granola, 500 g (7.50), Notes on your order (long answer), Delivery or pickup (one of 2), Delivery address (address), When do you need it (date) and Preferred time (dropdown).
“Delivery address” only appears when someone answers “Delivery” to “Delivery or pickup?”.
It ends with a thank-you message you can edit.
After someone submits
A Slack post. Every submission is posted to #orders in your workspace, with the answers that matter pulled into the message, so the team sees “Order form” responses where they already are.
The workflow copies with the form, switched off until you have filled in the blanks. The form itself publishes on the free plan; switching an approval on is a Pro feature, and a Slack post needs your workspace connected once.
Make it yours
The options under “What would you like?”. are placeholders: put your own in. Add, remove or reorder questions in the builder; every change is tracked and nothing goes live until you publish. Pick a design preset or your own colours, add your logo, and share the link or embed the form on your site.
