A friendly RSVP for a party: coming or not, a plus-one, what they can eat, and a message for the host.
What is in the form
Your name, Can you make it (one of 2), Bringing a plus-one (yes or no), Who are you bringing, Anything you don’t eat, One song that has to be played and A message for the host (long answer).
“Bringing a plus-one?” only appears when someone answers “Yes, count me in” to “Can you make it?”. “Who are you bringing?” only appears when someone answers “yes” to “Bringing a plus-one?”. “Anything you don’t eat?” only appears when someone answers “Yes, count me in” to “Can you make it?”. “One song that has to be played” only appears when someone answers “Yes, count me in” to “Can you make it?”.
It opens with a welcome screen (“You’re invited!”) and ends with a thank-you message you can edit.
After someone submits
This template comes without a workflow. Responses land in your dashboard, and you can add a step in the workflow editor: an approval by email, a post to Slack or Teams, a row in Notion or Airtable, or a webhook.
Make it yours
