Sports club membership
Register new members for a sports club: player details, a parent or guardian for juniors, medical notes, consents, and the membership secretary's approval.

What is in the form
Page 1: Member details. First name, Last name, Date of birth (date), Phone, Email, Home address (address), Membership type (one of 5) and Which team or section (dropdown).
Page 2: Safety and consent. Parent or guardian, Their phone, Emergency contact, Their phone, Medical conditions, allergies or injuries the coaches should know about (long answer), Previous club, if any, I agree to follow the club’s code of conduct (yes or no) and May we use photos of matches and training that include you on the club’s website and social media (yes or no).
“Parent or guardian” only appears when someone answers “Junior (under 18)” to “Membership type”. “Their phone” only appears when someone answers “Junior (under 18)” to “Membership type”.
It opens with a welcome screen (“Join the club”) and ends with a thank-you message you can edit.
After someone submits
An approval by email (Pro). Each submission is emailed to an approver you name, with the answers and two buttons: “Approve” and “Decline”. The decision is recorded on the response, and the rest of the workflow can branch on it.
The workflow copies with the form, switched off until you have filled in the blanks. The form itself publishes on the free plan; switching an approval on is a Pro feature, and a Slack post needs your workspace connected once.
Make it yours
The options under “Which team or section?”. are placeholders: put your own in. Add, remove or reorder questions in the builder; every change is tracked and nothing goes live until you publish. Pick a design preset or your own colours, add your logo, and share the link or embed the form on your site.
