Sharing a form

Once a form is published, Distribute has everything for sharing it.

  • Share link: anyone with it can open and submit the form. Copy it or Open form to try it.
  • QR code: Download it as an image for posters, flyers or a screen.
  • Share on social: X, LinkedIn, Facebook or email. The preview uses the form’s title, the welcome screen’s text when there is one, and your cover image.
  • Embed: code to paste into your site. It shows the form without its page background, so it blends in. It’s a regular <iframe>: change its height to fit the form. In Notion, use /embed with the share link.

When the form is switched off, closed or not open yet, the page says so; the link starts working again when the form reopens.

On your own domain

With a custom domain (Pro), forms are served from an address like forms.yourcompany.com, and Custom link gives a form a readable address there, like forms.yourcompany.com/quote-request. The original link keeps working.

Add ?key=value to the link to fill in questions, or hidden fields that record where the respondent came from. See Question settings.
